Confidentiality Policy
A Better Life 2 Embrace’s workforce members, contract personnel, volunteers, professional service persons, suppliers and accrediting bodies shall not convey to any person outside of the corporate entity that an individual attends, nor disclose any information identifying a consumer/client is/has received services for mental health, substance abuse or any other service from A Better Life 2 Embrace without written consent for release of information (PHI-Protected Health Information) per our corporate entity policy. This type of disclosure is only authorized by court order, to qualified personnel for medical emergency, research, audit or program evaluation purposes.
Federal laws and regulations do not protect any threat to commit a crime; any information about a crime committed by a consumer/client either at the corporate entity, or against any person who works for the corporate entity will be reported to the proper authorities.
This information has been disclosed to you from records protected by federal confidentiality rules. The federal rules prohibit you from making any further disclosure of this information unless further disclosure is expressly permitted by the written consent of the person to whom it pertains or as otherwise permitted by 42 CFR, Part 2. A general authorization for the release of medical or other information is not sufficient for this purpose. The federal rules restrict any use of information to criminally investigate or prosecute any alcohol or drug abuse client. Drug abuse patient records are also protected under HIPPA 45 CFR, Part 160 &164 and apply to every page disclosed.
